Company Licensing and Amendments
Users of the NAIC’s Uniform Certificate of Authority Application (UCAA) application should read and review the UCAA instructions, FAQs and state charts prior to starting a UCAA application. This information and the application can be found on the NAIC's UCAA website.
- All Life & Health insurers, Property & Casualty insurers, Fraternal Benefit Societies and Title insurers seeking initial licensure in Virginia must use the appropriate UCAA application. Obtain the most current version of the NAIC biographical affidavit form from the UCAA website.
- Use the UCAA Corporate Amendments Application to add or delete lines of authority.
- Mergers, name changes, and redomestications require the UCAA Corporate Amendment Application to be filed with the NAIC and appropriate documentation to be filed with the Clerk’s Office

In accordance with Chapter 12 of Title 38.2 of the Code of Virginia, Reciprocal insurers seeking licensure in Virginia shall file with the Bureau of Insurance a written power of attorney appointing the Clerk of the Commission as agent of the Reciprocal.
